🧭 Levels of NDIS Support Coordination
There are three levels of NDIS-funded support. Here’s how they differ:

| Support Connection | Entry-level support designed to help build skills to connect with community, mainstream, and funded supports. | – Light guidance to get started – Finding services – Managing your plan |
| Support Coordination | For participants with more complex goals or needs. Includes all aspects of Support Connection, with added support. | – Coordinating multiple providers – Crisis support and response planning – Helping you understand and use your plan – Building independence over time |
| Specialist Support Coordination | For participants facing significant risk, crisis, or involvement with multiple service systems. | – Managing numerous high-risk needs – Navigating justice, child protection, or mental health systems – Handling crises or disruptions in care – Complex needs |
Please note: We don’t currently provide Specialist Support Coordination services, but we’ll connect you with trusted providers who do.

The easiest way is to check your NDIS plan under Capacity Building Supports. You’ll be able to see if Support Connection or Support Coordination is listed.

4. Support for Complex Situations
For participants with higher or complex needs, such as managing multiple health conditions or requiring extensive support for daily living, we help coordinate providers, informal support networks, and mainstream healthcare to ensure services work together to support their well-being.
5. Capacity Building
Our goal is to empower you. We support you in growing in confidence, learning to manage your plan, and becoming more independent.
